🛟Need Help? How to Use Clearance the Portal & FAQ page

  • Step 1: Create an Account and Complete the Log-in Process. THIS IS WHERE YOU WILL SELECT ALL ACTIVITIES THE STUDENT WILL TRYOUT FOR DURING THE ENTIRE SCHOOL YEAR.
  • Step 2: Fill Out All Sections and Online Forms. Upload a CURRENT physical when prompted with a Physician’s stamp on the form (MD, or DO only).
  • Step 3: Complete the Online Signature Agreements.
  • Step 4: Print and sign the Confirmation page. Parent and Student must sign. You can print from the site, from the email that is sent or take a screenshot and sign on your phone.
  • Step 5: Email the signed Confirmation to granitehillsathletics@guhsd.net

This process must be done to participate in any sport. Physical is valid for one calendar year, and is kept on file.

INSURANCE:

  • All students clearing for extracurricular activities that require a clearance MUST have current health insurance to try out and participate. Do not leave the Insurance Company and policy number blank on the clearance process. Students will not be cleared if this information is not provided.

PHYSICALS:

Physical Form: Download the form

Step 1: Parent completes and signs the top section of the form prior to doctor visit.

Step 2: Doctor (MD or DO only) must stamp & sign the completed physical form.

Step 3: Upload valid physical when prompted when completing online clearance. Physicals are required annually and must be valid for the entire season of the sport.

Physicals are good for one year from the date of the exam – not the date the physician completed the form. Physicians must be licensed by the State of California.
